  5. Affidavit of Support from parents/ guardian (Authenticated by the Philippine Embassy in the place of origin)
  6. Bank Certificate (English version)
  7. Affidavit of Guardianship Transfer (Authenticated by
    the Philippine Embassy in the place of origin)
